SUMMARY

Builds and maintains relationships with community providers, provides support to patients and families to improve health status through monitoring of diabetic eye measures and education. Responsible for triaging acute/chronic eye disease referrals and scheduling accordingly. Supports community and internal group education sessions. Responsible for the database management and HEIDS quality measurement reporting.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Communicates directly with patients who are referred via CCD to ensure patients follow-up on referrals and appointments. This includes phone contact, email and direct mailings.
  • Documents results appropriately within the Electronic Health Record. Coordinates as necessary with the PCP to ensure results are communicated.
  • Tracks and maintains current register of all CCD referred patients within the Electronic Health Record and ensures all data is accurately loaded.
  • Serves as liaison with network Optometrists collecting both billing and clinical data.
  • Assists the Diabetic Coordinator as needed with community events.
  • Assists with the management and internal functionality of the integrated eye network.
  • Participates in team/staff meetings; attends in service meetings and applicable workshops.
  • Other duties as assigned.

EDUCATION AND/OR EXPERIENCE

  • High School Diploma or Equivalent.
  • Working knowledge of prevention, diagnosis and management of Diabetes Mellitus Type 2.
  • Proficient in Microsoft Office applications.
  • Experience managing databases, health record database management preferred.
  • Ability to participate in team/staff meetings; attend in-service meetings and applicable workshops.
