Essential Duties and Responsibilities

The Family Nurse Practitioner will work with the Lead Provider to play a critical role in both care delivery as well as the ongoing design. This position will Float between all 3 Tucson practice locations. Specific tasks will include:

  • Delivering Primary Care within the practices, through both traditional face-to-face visits as well as non-visit based interactions (e.g. phone, email, groups, home visits, video chat).
  • Collaborating with a team of health coaches, who, based on a community health worker / promotora model, will help patients to set self-management goals, improve patient self-efficacy, and support physicians.
  • Participating in development of robust group visit offerings, including educational classes, integrative yoga and fitness classes, etc.
  • Taking a population management approach to care by viewing the entire community of patients as recipients, and using technology and team resources to direct care towards those most in need.
  • Participating in communicating and marketing the practice model to new patients as well as other stakeholders such as the medical community, payers, and the press.
  • Carrying out other appropriate professional duties as may be necessary, including liaison with outside medical consultants, hospitalists, emergency room providers, etc.

Qualifications 

We are looking for someone with 2+ years of experience treating adults in a Primary Care setting who has or can obtain a Arizona State license. The ideal candidate will be an excellent clinician, a systems thinker, technologically savvy, personable, be able to think out of the box, and someone who values customer service and efficiency highly. We want someone who is idealistic, thoughtful, articulate, hard working, creative, flexible, and able to deal with uncertainty. Candidates with work experience outside of medicine, and those with additional advanced degrees or specialization are encouraged to apply.
