Job Description

Reporting to the Clinical Team Manager, the Registered Nurse (RN) serves as a key member of the Iora Health team and plays a critical role in helping patients improve their health.  The RN engages patients and encourage them to take an active role in their health by providing them with the tools necessary to make healthy lifestyle choices and adopt life-long healthy behaviors. Our RN's help patients unlock personal motivation to make lasting changes and simultaneously serve as the patients’ primary conduit to the practice as they help coordinate necessary services both within and outside of the practice.

In addition to their active clinical involvement, Iora RN's rotate through the roles of greeting patients to the practice and providing patient support wherever needed, allowing them to influence the patient experience in its entirety, from the moment patients set foot in the practice to the time they leave and well beyond. We believe understanding and fostering the continuity of the relationship helps build the strength of the collaborative culture Iora Health embraces.

Specific RN responsibilities include, but are not limited to the following:

  • Collaborating with colleagues to actively engage patients in their daily care
  • Communicating with patients in person, in groups, by telephone and via electronic means
  • Implementing a range of interventions/skills including counseling, motivational interviewing, coaching, education, referral and advocacy to gain access to services and resources for patients
  • Providing patient education on on chronic disease management on which the health coach will be trained
  • Communicating care plans and pertinent information with the internal practice team as well as with other parties participating in the care of the patient
  • Assisting with the coordination of care across the care continuum (including home care, outpatient care, ER care, and hospital care) while maintaining strict patient confidentiality at all times

RN Candidate Attributes:

  • 3+ years as a Registered Nurse required
  • Current WA License required
  • Experience in a service environment required
  • Must be open-minded, non-judgmental and compassionate
  • Must demonstrate excellent interpersonal communication skills including exceptional listening skills
  • Must have exceptional organizational, problem-solving and time management skills
  • Must demonstrate intellectual independence and willingness to learn new skills
  • Must be flexible and comfortable with ambiguity
