As a home care Physical Therapist, you will be a key part of a multidisciplinary team delivering high-quality rehabilitative care and skilled nursing. Your work will empower clients to recover safely at home while improving their mobility, independence, and quality of life.

What You'll Do:
- Perform home visits and develop personalized treatment plans
- Conduct diagnostic tests and assessments (mobility, range of motion, strength, balance, etc.)
- Implement therapeutic exercises, gait training, and balance interventions
- Educate clients and caregivers on rehabilitative care techniques
- Maintain accurate documentation using a state-of-the-art tablet-based EMR system
- Collaborate with nurses and other healthcare professionals to ensure comprehensive patient care
What We’re Looking For:
Current North Carolina Physical Therapist (PT) license
Strong interpersonal, organizational & problem-solving skills
Ability to work independently & manage time effectively
