Associate Medical Director - High Risk Care

Iora+ is seeking a part time Medical Director (FTE 0.4 - 0.5) to join our High Risk Care team in the Phoenix, AZ; Washington, and Georgia markets. The High Risk Medical Director will serve as a key member of the High Risk Care team, playing a critical role in guiding the clinical management of our high risk/high need patients. This role reports to the Medical Director, High Risk Care

Position Highlights:

  • People management of 3-4 NPs in Phoenix market, plus potential to support in Georgia (ET) vs Washington (PT) depending on time zone.
  • Attend and collaborate on morning huddles daily to guide clinical plan of care 
  • Attend and participate in weekly multidisciplinary team meetings and continuing care rounds 
  • Collaborate with Nurse Practitioners on plan of care for patients seen on a weekly basis through case review on a scheduled and as needed basis, including calls to Specialists and hospitalists 
  • Chart review for clinical quality improvement. Create and implement quality improvement for Iora+ team cross-market 
  • Partner with Iora+ Medical Director on program development and clinician education 
  • Build relationships with key external stakeholders (SNFists, Hospitalists) 
  • Opportunity to collaborate with Clinical Performance and Strategy team on other key population health projects including Clinical Advisory Team and provider education 
  • Cover clinical care when needed due to PTO and absences 
  • Provide direct care to Iora+ patients as needed based on acuity

Qualifications:

  • Must be a licensed physician with an active AZ, WA, or GA license
  • Minimum 3 years experience with supervising providers, including NPs
  • Experience with team based care, excellent collaboration skills 
  • Deep experience with high risk, complex care
  • Able to teach and model Geriatric and Palliative-focused care
